OUR PROGRAM
To prevent chronic kidney disease or slow down its progression into the later stages. The curriculum was developed using material from the American Kidney Fund, Baxter, Fresenius, and the National Kidney Foundation.
SERVICES
What We Offer
- Classes offered once a month
- Education on preventing and managing chronic kidney disease (CKD)
Who Is Eligible
- Class I: Open to the public
- Class II: For patients with an eGFR of 25–45 (provider referral required)
- Class III: For patients with an eGFR of 24 and below (provider referral required)
