OUR PROGRAM

The Healthy Outcomes for Maternal and Early Childhood (H.O.M.E.) Visiting Program is a free, voluntary, in-home service designed to support families through the joys and challenges of pregnancy and early parenting.

Through home visits, an assigned Community Health Worker (CHW) works with moms, dads, caregivers, and their families to provide guidance, resources, and encouragement every step of the way.

  • Personalized support in the comfort of your home, meeting families where they feel most at ease
  • Flexible home visits (weekly to quarterly) tailored to your family’s needs and schedule
  • Guidance on infant and early childhood development, using proven, family-friendly curriculum
  • Help connecting to community resources, so families don’t have to navigate services alone
  • Early childhood developmental screenings to support healthy growth and learning
  • Support with preventive check-ups, including:
    • Transportation vouchers when needed
    • Assistance scheduling medical and developmental appointments

Who Is Eligible?

  • Pregnant women, parents, or primary caregivers with a child under three months of age
  • A screening and eligibility assessment is conducted to determine program eligibility

How to Enroll in Home Visiting Services

  • Submit a Referral
    Families can be referred by a healthcare provider, community partner, or by reaching out directly to the program. Once the referral form is received, our team begins the enrollment process.
  • Connect With Our Team
    A friendly program staff member will contact the family to complete a brief, two-step screening using the Healthy Families America (HFA) screening tool.
  • Family Assessment
    Families who meet the initial screening criteria will complete the Families Resiliency and Opportunity for Growth (FROG) assessment with trained staff. This helps us better understand each family’s strengths and needs.
  • Determine Eligibility
    If the family is eligible, home visiting services may be offered right away.
  • Enroll and Get Started
    Families who choose to participate are assigned a dedicated home visitor. The home visitor schedules the first visit and helps the family complete enrollment forms so services can begin.
