Programs and Services
OUR PROGRAM
The Diabetes Self-Management Education & Support program helps individuals and families prevent and manage diabetes and hypertension through education, support, and community engagement. The program promotes healthy lifestyles, builds skills for daily self-care, and connects participants to resources and services. By working with community partners and supporting environmental and policy changes, the program strengthens long-term prevention and improves health outcomes across the CNMI.
Diabetes Self-Management Education & Support
- Weekly sessions on diabetes self-management education and support
(after-hours sessions available from 5:00 PM – 6:00 PM). Topics include basic nutrition, foot care, medication compliance, and physical activity - Support for creating personalized self-management goals along with progress and encouragement check-ins to help participants stay on track
- Referrals for accessing community resources
Who Is Eligible?
- Individuals ages 6 and above diagnosed with diabetes with an A1c reading of 5.6 or higher
- Pregnant women diagnosed with gestational diabetes
