📞 Answer the Call. For Our Families. For Our Future.

Beginning in mid-September, the CNMI Health Survey will ask adult CNMI residents about their health and well-being. For the first time, the survey will be conducted as part of the CDC’s Behavioral Risk Factor Surveillance System (BRFSS), the nation’s largest ongoing health survey.

Residents will be randomly selected to participate. If selected, you may receive a call from one of these authorized numbers:

📱 (808) 547-2941
📱 (808) 547-2942
📱 (808) 547-2943

Participation is voluntary and confidential. Every response helps provide a clearer picture of the health of our islands and ensures that communities throughout the CNMI are represented.

💙 Your voice matters. Help us build a healthier future for the CNMI.

For questions or to verify a survey call, contact the CHCC Division of Public Health Services at (670) 236-8723 or (670) 236-8222. If you want to know more about the CNMI Health Survey, visit https://phs.chcc.health/cnmi-health-survey/ for more information.
