Community Health Assessment (CHA)

The Community Health Assessment (CHA) provides a snapshot of the community’s strengths, needs, and priorities. It is the result of collaboration with community stakeholders to assess community health in the Commonwealth of the Northern Mariana Islands (CNMI) using a process called Mobilizing for Action through Planning and Partnerships (MAPP).

This CHA report serves as the foundation for developing a Community Health Improvement Plan by identifying evidence-based interventions and guiding where resources should be allocated to best meet the community’s priority needs.


Community Health Improvement Plan (CHIP)

The Community Health Improvement Plan (CHIP) is a roadmap for improving community health and wellness by addressing priority issues identified in the CNMI Community Health Assessment (CHA) 2024 Report.

Together, the CHA and CHIP address the social determinants of health and focus on eliminating root causes and behaviors that impact community health. The recently completed CNMI CHA provides a snapshot of our communities’ strengths, needs, and health priorities and serves as the foundation for developing this CHIP.
